The Welsh Renal Clinical Network need your help.
The Welsh Clinical Renal Network are looking to introduce a new form to document an individual’s wishes. They are in the process of creating the new document titled ‘My Life, My Wishes’. This document is intended as an Advance Care Plan (ACP) or Statement of Wishes by adults who can make their own decisions. The document is still in the development stages, with the end result acting as a record of your wishes about how you would like to be cared for in the future.
Your feedback will help with the finalisation of the text and content. Please take a moment to review the English version of the PDF, linked below, and provide feedback using the form at the bottom of this page. A Welsh language document will also be available.

Having a record of your wishes will provide ‘loved ones’ and ‘healthcare professionals’ with valuable information of ‘what is important’ to the you and other individuals at what is often a very difficult time.
The introduction of this information will help your loved ones and health and care professionals involved in your care and treatment.
